Monthly Forms to Earn PTO

IMPORTANT NOTE: Due to the current furlough of the majority of DECC staff and to remain in compliance with the law, several areas of the Wellness Program (including the Wellness PTO Program) will be suspended, effective beginning at 11:59pm on April 30, 2020, until further notice. All points accumulated through April 2020 will not be affected and will be put in a “holding” status. This website will continue to be updated with information and resources for DECC employees. THE 2020 WELLNESS PTO PROGRAM

HOW IT WORKS:

  • Beginning January 1st, 2020, you can use this form to track your daily habits:

  • Participate at any time between January 1st and November 30th, 2020
  • Track your habits using the form to see how many points you can earn each day/month.
  • Check the post above to see which “pop-up points” you can earn that day. (These will be updated randomly throughout the year.)
  • Collect 200 points to earn 1 hour of PTO (paid time off). You can earn up to a full 8-hour day!
  • Submit your form by the 5th day of the following month. You can drop your form off at the DECC Business Office, submit electronically at deccwellness.org/pto or email it to wellness@decc.org
  • We keep track of your points for you and we will let you know when you reach 1600 points to earn your 8 hours of PTO!
  • If you have not earned your full day by Nov. 30, 2020, we will send you an email to let you know how much you have to “cash in”.

 

MORE DETAILS:

  • 200 points = 1 hour of PTO; up to a maximum of 8 hours
  • All DECC employees are eligible: full-time, part-time, and seasonal
  • Forms must be submitted by the 5th day of the following month.
